The five confined space permit mistakes we see most on Australian sites

Most confined space near-misses trace back to paperwork, not equipment. Here is what our trainers find when they audit live permits.

Under the WHS Regulations, a confined space entry permit is the control that ties every other control together — atmospheric testing, standby personnel, rescue arrangements and isolation. When our trainers review real permits brought in by clients, the same five gaps appear again and again.

First, the space is not actually classified. Teams treat a pit, tank or silo as "tight but fine" without applying the regulatory test of restricted entry, inadequate ventilation and a risk of harmful atmosphere. Second, gas testing is recorded once at the start of the shift rather than continuously, so oxygen displacement from purging or hot work goes unnoticed.

Third, the standby person is given other duties. A standby who is also fetching tools or filling out paperwork cannot maintain continuous communication. Fourth, rescue is described as "call 000" — which is not a rescue plan, because emergency services cannot be relied upon to perform a vertical retrieval within the survivable window. Finally, permits are not closed out, so nobody confirms the space is clear of people and equipment before it is sealed.

How often does working at heights training actually need refreshing?

There is no legislated expiry on RIIWHS204E, yet most principal contractors will turn you away after two years. Here is why.

Working at heights competency does not carry a statutory expiry date the way a high risk work licence does. What the WHS Regulations require is that a worker is competent for the task at the time they perform it — and that the person conducting the business can demonstrate that competence.

In practice, industry has settled on a two-year refresher cycle. Tier-one construction, rail and resources clients almost universally specify it in their contractor management systems, and insurers increasingly ask for evidence of the same. If your Statement of Attainment is older than two years, expect a site induction to stop at the gate.

Refresher training is also where genuine skill decay is caught. Anchor point selection, fall-arrest system clearance calculations and suspension trauma response are perishable skills. Workers who have spent eighteen months on scaffold rather than in a harness routinely mis-select connectors on their first refresher exercise.

Our recommendation to safety managers is simple: build a two-year rolling matrix, and bring forward a refresher whenever a worker changes fall-protection systems, returns from extended leave, or is involved in a near-miss at height.

On-site or public course? A practical way to decide

The break-even point is usually smaller than managers expect — and downtime, not course fees, is where the real cost sits.

When a client asks whether to send people to our Sandgate facility or bring us to them, the honest answer is that it depends on headcount, travel and how site-specific the hazards are.

Public courses win for one to three participants, for workers who need a specific scheduled date, and where practical assessment requires purpose-built infrastructure — vertical rescue towers, confined space simulators or live-fire props. Our calendar runs continuously, so a single new starter can be certified without disrupting a crew.

On-site delivery wins from roughly four participants upward, and the calculation is dominated by lost productive hours rather than course fees. Travel time, vehicle costs and the ripple effect of pulling a crew off a job for two days usually exceeds any per-head saving. On-site training also lets us assess workers against their own plant, their own permits and their own confined spaces, which is far more defensible in an incident investigation.

The hybrid approach many of our long-term clients use: run the bulk of the workforce on-site annually, then top up new starters through public courses between cycles.

What a genuinely ready emergency response team looks like

An ERT on paper and an ERT that can retrieve a casualty in eight minutes are very different things. Four tests to tell them apart.

Most sites we visit have a nominated emergency response team, a wall chart and a set of equipment lockers. Fewer can demonstrate that the team will function under load at 4am with two members on leave.

The first test is depth. If your response capability collapses when any single person is absent, you do not have a team, you have a dependency. We recommend training to at least double the minimum roster requirement.

The second is unannounced drills. Scheduled drills validate equipment; unannounced drills validate decision-making and communication. Time to first competent responder on scene is the single most useful metric a site can track.

Third, equipment familiarity. Breathing apparatus, tripods, winches and stretchers must be handled by the people who will use them, at the frequency they will use them — not once at induction. Fourth, integration with external services: someone must own the handover, the site access details and the exact grid reference emergency services will need.
